The Assisted Living Community / Adult Family Home
The Care Solutions care manager has valuable experience in knowing about the communities of assisted living and their particular specialty. The recommendation to the community is not tied to a referral fee but rather to the needs assessment from meeting the older adult. Whether it is relocation for a permanent home, respite care for a short stay due to illness or ongoing needs for temporary care your care manager will be available to facilitate the move. The Care Solutions care manager will be your family’s advocate to review and monitor care if this is your family member’s permanent home.

- Assistance with locating best care community for level of care
- Review of cost of care
- Professional Negotiated Care Plan provided for relocation into Adult Family Home; Mandated by Washington State.
